Exceptional Family Member Program

The Exceptional Family Member Program is a mandatory enrollment program (MCO P1754.4A) for all Active Duty personnel. The program is designed to provide assistance to service personnel with a family member(s) who has special medical and/or educational needs before, during and after relocation due to Permanent Change of Station (PCS) orders. An exceptional family member is defined as an authorized family member (spouse, child, stepchild, adopted child, foster child, or a
dependent parent) residing with the sponsor who may require special medical or educational services based upon a diagnosed physical, intellectual or emotional handicap such as Asthma, Cerebral Palsy, Mental Retardation, Dyslexia, ADD, ADHD, Autism, Oppositional Defiant Disorder, or Depression. Disabilities may range from mild to severe.

Your local EFMP Coordinator can provide you with the following information:

  • Information about local support groups.
  • Points of contact for: Tricare Region and local Tricare Service Center, Local school district special education offices, to include DoDEA, DDESS and DoDDS schools.
  • Information about any special programs, services and adaptations that are unique to the local installation and surrounding community.

Housing

A sponsor enrolled and assigned an EFMP Category 4 is entitled to priority assignment to government housing. If a sponsor enrolled in the Exceptional Family Member Program resides in government housing, and requires structural adaptations to the government house, the adaptations will be made in compliance with PL 93-112 and DOD 1020.1 when necessary to meet the special needs of the family member.

  • What are the enrollment category definitions?
    After your completed EFMP medical form DD 2792 and/or educational form DD 2792-1 have been processed, you will receive
    a letter from the Headquarters Marine Corps which will contain one of the following categories: Category 1: Needs generally
    do not limit assignment. Category 2: Pinpoint assignment overseas and within the Continental United States. Category 3: No overseas assignment. Category 4: Major medical areas within the Continental United States.

  • How do I enroll in the EFMP?
    1. Ensure your family member is enrolled in DEERS.
    2. Contact your EFMP Coordinator at Marine Corps Community Services who will assist you in obtaining medical and educational evaluations, and provide the necessary forms for you, your medical provider, and, if necessary, your child's school official to complete.
    3. Return the completed forms to the EFMP Coordinator who will forward them to the Central Screening committee and to Headquarters Marine Corps for processing and category assignment.
  • How long may I be enrolled in the EFMP?
    For as long as necessary. Disenrollment from the EFMP requires a letter from a medical doctor stating that services are no longer needed. Disenrollment may be prompted by a change in marital status, a change in the dependency status of enrolled family member or a change in the medical status of the enrolled member.

  • What are my responsibilities?
    Required to update your enrollment every two years by completing DD 2792 and/or DD 2792-1 submitting the form to your EFMP coordinator.
